Please sign up for our Christiana Care Patient Portal.

• The Portal gives you access to your information and a way to communicate with our office without having to us the phone system.
• The Portal gives you access to your lab results, the ability to request refills or an appointment or to ask general questions to anyone in our office.
• The Portal Allows you to print lab slips, radiology slips, medical equipment orders (DME) and physical therapy orders right from your Portal Home Page.

All we need is your current email address and we will register you for the CCHS Patient Portal.  Make sure our office has your email address in our system.
